NAIC
NAIC requires a biographical affidavit must be completed and an independent third party verification report (background investigation) be completed for all Officers, Directors or Key Managerial Personnel in connection with a UCAA application for licensure. The NAIC Biographical Affidavit is used to evaluate the suitability, competency, character and integrity of the Affiant.
The independent third party verifier must be vetted and approved by NAIC. INQUIREHIRE is an NAIC approved vendor and has been providing these services since 1994.
GETTING STARTED (5 easy steps)
*Review, sign and send in the service application and service agreement.
*Forward the biographical affidavit(s) via secure link, secure email, FedEx, or fax.
*Inquirehire performs the background investigation.
*Final report sent to the state(s).
*Invoice for service will be sent.
IMPORTANT CONSIDERATIONS
*Make sure you are using the most recent version of the biographical affidavit (Form 11). Most recent version has a revision date of 12/08/2020.
*All questions on the affidavit must be answered, “none” or “not applicable” can be used if needed.
*All disclosures/authorization forms must be signed, dated and notarized with the last 6 months.
